Medical Case Management Services     

Case Management is a tool used to ensure cost effective, quality medical care and to assist in timely resolution of a claim.  Case Management can be on site or telephonic.  A Nurse Case Manager's role is to serve as a liaison to enlist the cooperation of the injured worker, physician and employer.

C.A.R.E. performs general medical case management through attendance of doctor's appointments, direct contact with patients and updates from services, when permitted.  Staff are available to provide medication and cost analysis, coordination of home and transportation modifications when necessary to accommodate the client.

Vocational Rehabilitation Services     

The primary goals of Vocational Rehabilitation involve returning the client to employment in the same or modified job, returning the client to employment capitalizing on transferable skills, or returning the client to work following training.

A Vocational Rehabilitation evaluation focuses on the residual functional capabilities, intellectual and academic abilities, employment history, and interests of the client to determine transferable skills and what training, if any, is appropriate to return the client to gainful employment.

C.A.R.E.'s certified evaluators provide services including:
  • vocational rehabilitation evaluations.
  • vocational case management/coordination.
  • job placement assistance with or without on-the-job training.
  • labor market surveys.
  • file analysis. 
  • expert witness testimony. 

Vocational case management includes providing monthly progress reports to all parties as well as providing support and assistance to facilitate the client's success during educational endeavors.

Life Care Plans     

The Life Care Plan calculates approximate needs, both medical and non-medical, of the person with a devastating injury.  The Plan provides an estimation of goods, services and equipment, along with maintenance and replacement, needed by the individual over a projected life expectancy.

Catastrophic life care plans can be provided by qualified professionals at the C.A.R.E. office.  In addition, long and short-term care plans can be accomplished.  The staff at C.A.R.E. is also available to assist with Medicare set asides and cost analysis of both medications and procedures.
